NOTIFICATION OF PASSPORT REQUIREMENT FOR US DELEGATES TRAVELING TO TORONTO
Please note the following Visa Information:
A US passport is required for reentry into the US from Canada. For decades all that was required was a US birth certificate and while that changed several years back, those who have not travelled there for some years may be caught off guard. Getting a US passport can take up to 5 or 6 weeks.
To determine which fee type to select (as defined according to the World Bank Country Classification list of countries)
A3 - High income & Upper-middle-income countries
A1 + A2 - Low-income & Low-middle-income countries
* Including psychologists, nurses and social workers excluding fellows and registrations supported by Industry
Applicants for student- in order to benefit from the special fee, a submission of your status confirmation (approval letter signed by the Head of Department or copy of your status ID) must be uploaded during the online registration.
** For Patient/Advocacy Rate, you must indicate your affiliation in the registration form. Registrants in this category are ineligible to claim CME credits.

FEES FOR PARTICIPANTS INCLUDE:

All participants must submit a completed registration form. Payment or proof of payment should accompany the registration. Please make a photocopy of your registration form for your own files.

Important: All bank charges are the responsibility of the participant and must be paid in addition to the registration fees.
Please remember to specify the name of the Congress and the participant on the bank transfer document.
If payment is made by a company or for more than one person, please make sure all names are indicated.
The completed registration form together with the relevant payment must be received by March 16th, 2015, in order to benefit from the reduced registration fee. Thereafter the registration fees will increase as published.
REGISTRATION CANCELLATION POLICY
All cancellations must be electronically mailed. Note, in case of cancelation at any stage, Bank transfer admission fee (30 USD) will not be refunded - applicable to Bank Transfer payments only.
Refund of registration fee will be as follows:
- Cancellations received up and including March 16th, 2015 - Full refund
- From March 17th, 2015 – No refund
Group Registration procedure will be valid for 10 participants and more.
